HELP- Where do I get the script for my Form to work? I'm using Golive

I've made a website for someone, and this is where he's going to be hosting his site. Im new to making websites, and this is the first form I've done. From what I'm hearing is that I need to get a script from the host (which is here, discountasp) and they'll tell me what I need to do with it and where to insert the correct information. I wasn't able to find a phone number to call where I could talk with someone- So i turned here.

Does that link you sent me to have all the instructions for me to follow to make it work, or will I need to 'know' to do certain things. I say this because i saw that it said this 'Keep in mind that this script will require some customization in order to work on any particular site. You may also wish to alter it to suit your needs better, such as by reformatting the output or adding new options.'

you need some basic knowledge on ASP but the instruction should be simple enough
