Avatar Image never works, and the button is the wrong style?

Hi,

I've got my website based on the Club Kit setup at http://www.skylinecheer.com/ however when I try to upload an avatar image I don't think it ever uploads. The thumbnail is the red X meaning the image isn't found. Also the buttons on the page are 'old windows style' not 'new windows style'. When I try the app locally I get the same problem, except the buttons are 'new windows style.'

Any ideas?
